Online Consultations

If you have a routine question or a query about a health related issue you can submit an online consultation via Accurx which is the practices patient triage service. Please click on the image opposite.  You will receive a response to your query as soon as we are able to.

Urgent Appointment

To request an urgent appointment for today or tomorrow (Monday to Friday):

  • phone us on 01623 664877, Monday to Friday from 8am

When you get in touch, we’ll ask what you need help with.

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

Routine Appointment

To request a routine appointment please contact us using the following options:

  • use our online system via the link at the top of this page (click here to see what type of appointments are now available to book online)
  • phone us on 01623 664877, Monday to Friday from 8am
  • visit the surgery and speak with a receptionist, Monday to Friday from 8am
  • use your NHS account (through the NHS website or NHS App) to book a screening test or vaccination

 

If you are you constantly disappointed by not being able to get a GP appointment you can help the practice and the whole NHS to try and solve this problem by choosing the appropriate NHS Care. If you are unsure what service is best for your health care need click here for more advice.

Extended access

Churchside Medical Practice work in collaboration with Roundwood Surgery, Acorn Medical Practice, Millview surgery and Forest Medical Practice to provide GP & Nurse Appointments Monday to Friday 7am-8pm and Saturday 8am-5pm. 

This means if you require a non-emergency, pre-booked appointment when we are not open you may be able to see a GP or Nurse at one of the practices listed above. You can book these appointments up to 4 weeks in advance by contacting the practice you are registered with. Unfortunately, these appointments cannot be booked through the online system.

As part of this service Churchside Medical Practice offer GP and Nurse appointments until 8pm on a Wednesday evening and Nurse appointments only from 7.30am every weekday.

Home visits

If possible please try to telephone reception before 10am if you require a home visit.

A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.

House visits are only available for patients who are housebound because of illness or disability.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health Centre.

Your appointment

However you choose to contact us, we may offer you a consultation:

  • by phone
  • face to face at the surgery
  • on a video call
  • by text or email

Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.

Cancelling or changing an appointment

To cancel your appointment:

  • use your NHS account (through the NHS website or NHS App)
  • phone us on 01623 664877, Monday to Friday from 8am
  • reply CANCEL to your appointment reminder text message

If you need help when we are closed

If you need medical help now, use NHS 111 online or Call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

If you need help with your appointment

Please tell us:

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need an interpreter
  • if you have any other access or communication needs

Reception triage

Please be aware the receptionist will ask you the following questions when you are booking a GP Appointment.

  • Has the problem you are calling about come on in the past few days or is it a long-standing problem?
  • Do you mind me asking what the problem is please?

The reason for these questions is not because the receptionist is being nosey but to help determine the best possible pathway for your care, for example do you require a 999 ambulance, to see a GP or Nurse that day or later the same week.

In recent months our receptionists have undergone and continue to partake in detailed training provided by outside and internal sources to help them deliver a better quality of service for our patients. They are bound by the same Code of Confidentiality that ALL NHS Employees including GPs must adhere too, and if this is broken will receive the same formal disciplinary procedures.

The GP Forward Plan Produced by NHS England promoted this change in protocol and The GP Partners at Churchside Medical Practice Dr Vanessa Pearce, Dr Claire Harrison, Dr Wendy Barry, Dr Sukhvinder Dhamrait and Dr Vito Carone are in total agreement with the changes and hope it will result in a better quality of care for our patients.
